Record the medication list of an elder, disabled or chronically ill person (patient) that takes at least five (5) prescription
medications and at least 1 herbal supplement.

All demographic data
List all prescription medications & non-prescription medications/supplements.
Determine if there is any potential harmful prescriptions through review of the BEERS list and the “Checking for Possible Drug-Herb
Interactions” web page.
Review the BEERS List and denote with a checkmark if the medication is included on the BEERS List
Provide an analysis of potential interactions.
Suggest an alternative, but safer medication/supplement, keeping in mind that prescription medications are specific to the patient condition.
Provide two points of patient teaching related to this medication review.
