Anticoagulant in clinical practice:
This is the second part of my dissertation,
My dissertation topic is Anticoagulant safety in clinical practice. I should focus on the errors (incidents) of anticoagulant in
prescribing, dispensing, administration and monitoring only. According to the available statistical data by WHO, the NPSA, NICE, NHS,
Joint commission International(JCI) and Institute for safe medication practice incidents related to safety issues and errors in the
use of anticoagulant are still rampant.
Research aim
To examine the patterns, themes and trends related to anticoagulant drug incidents from the published literature and to compare and
contrast the findings with that from actual clinical incidents from the Cambridge University Hospitals NHS Foundation Trust (
Addenbrooke’s Hospital ) covering a two-year period.
Objectives
1. To determine or describe the most common themes of types of incidents involving anticoagulant.
2. To determine or describe the severity or clinical impact of incidents involving anticoagulant use.
3. To develop recommendations for reducing the number of anticoagulant use incidents.
4. To develop recommendations for reducing the clinical impact of anticoagulant use incidents.
Please include these following points in the literature review :
1- why I am doing the project (Justification) why it’s important?
a. Definition of patient safety and why its important ?
b. How many people are using anticoagulant in the world and in the UK (Important to know the number of people in the UK are using
anticoagulant and should be update number).
c. How many percentages of anticoagulants error or incident were dangerous from the NPSA, NICE, and NHS,Joint commission
International(JCI) and Institute for safe medication practice incidents, WHO.
d. how many people die due to the errors of anticoagulant in the world and particularly in the UK.
e. Economic issue (money) of anticoagulant medication error in the world ( very important to know the economic cost in the UK)
4- Definition of medication errors./ type of medication errorr
5- What is the database you are use for the literature review and the keywords.
6- What is the number of incident (percentage of errors) of anticoagulant from the literature review in each area: please add figure
or table
a- Administrating
b- Prescribing
c- Dispensing
d- Others
